Dear Member,  

We are pleased to offer you WHOLEHEARTED MINISTRY and its outreach Ministry services.  

Included is a private membership in WHOLEHEARTED MINISTRY Church and its  outreach Ministry and Memorandum of Understanding and in addition a waiver form  for the Private Health Association (PHA) who is assisting you by your specific  request. 

We thank you for your willingness to explore, and your choice to use the method of nature, and nature’s God for wellness and being whole as you were created to use and  be. 

WHOLEHEARTED MINISTRY is a lawfully formed, Church and outreach ministry based  on faith in nature and nature’s God, and Private association of the Church and its  outreach ministry to be natural and well. Our Church and its outreach Ministry, operates with a reliance upon our faith in Nature and Nature’s God, dedicated to providing  knowledgeable people offering their services and advice concerning natural wellness in  assisting its members. The following forms explain this relationship and the rights,  benefits and responsibilities associated with membership.

WHOLEHEARTED MINISTRY, A Faith Based Private Health  Ministry Membership Agreement/Waiver Form  

Overview: I do hereby apply for membership in The Church and/or its outreach ministry  WHOLEHEARTED MINISTRY, hereafter termed “The Ministry,” a private Church and  outreach Ministry. With the signing of this membership agreement, I accept the offer  made to become a member of The Church and/or its outreach ministry and have read  and agree with the following Declaration of Purpose, Preamble and Memorandum of  Understanding. 

Declaration of Purpose: The primary labor in the field of The Church and of its outreach  ministry of WHOLEHEARTED MINISTRY is focused on educating and serving  WHOLEHEARTED MINISTRY members so that they have all the latest knowledge in  providing protocols, including but not limited to Nature, Holistic Healing Practices,  Homeopathy, creation and/or distribution of supplements, tinctures, ointments, salves, or  any other natural form of wellness and peace; practice to protect and improve the  wellness of members; and provide access to WHOLEHEARTED MINISTRY protocols,  including but not limited to protocols that include homeoprophylaxis and other  homeopathic preparations. Secondary labor includes, but is not limited to: grassroots,  political, and economic efforts to ensure the protection of our labor in the field of Nature  and Nature’s God.  

Membership: By consenting to the Membership Agreement provided, members are  granted the following rights and benefits:  

  • Right to detailed informed consent about the labors rendered, and what to expect.  

  • Complaints against The Church and its outreach ministry WHOLEHEARTED MINISTRY member must be submitted in writing to The Church and its outreach  ministry; WHOLEHEARTED MINISTRY within 30-Days of the incident of  complaint. These will be sent to the man/woman, Committee, or Tribunal  designated by the Church and its outreach ministry; WHOLEHEARTED  MINISTRY for evaluation and remediation action.  

  • Access to the Church and its outreach Ministry’s Bylaws, and association papers  are available to candidates/members, upon request.

  • Guarantee that all Women’s Private and Personal Information (WPPI) remain in  the custody and in the confidential holding of the Church and its outreach ministry WHOLEHEARTED MINISTRY. Women’s private and personal Information of all  Members will be held in strict confidentiality and will remain private from any  State Medical Board, the FDA, FTC, Medicare, Medicaid or insurance companies  without the previously expressed specific permission granted by the Member and  deemed needful by the Church and its outreach ministry 

  • Membership with WHOLEHEARTED MINISTRY will remain intact unless the  member submits in writing their desire to withdraw membership or membership is  terminated by The Church and or it’s outreach ministry in accordance with bylaws.  

Preamble: We believe that the First Amendment of the Constitution of the United States  of America, organic, and the bill of rights and the Constitutions of the several states  guarantee our members the rights to religious liberties and freedoms and the rights of  free speech, petition, assembly, and the right to gather for the lawful purpose of advising  and helping one another in asserting our rights under law. IT IS HEREBY DECLARED that  we are exercising our right of “freedom of religion” and “freedom of association” as  guaranteed by the 1st and 14th Amendments of the Constitution of the United States of  America and all rites/rights known and yet to be discovered as well as its equivalent  provisions of the various State Constitutions, which are excepted out of the powers of  general government. This means that our Church and its outreach ministry activities are  restricted to private ecclesiastical jurisdiction and are private matters. We further  recognize that all rites/rights from the beginning of mankind to this day cannot ever be  unrecognized, or treated as if they were not so. We finally recognize that all rites/rights in  the People’s Covenant, sacred constitution, exist and the Church and its outreach  ministry and its members are absolutely able to freely exercise their chosen faith and our  public servants, as our government, created by we the people, are bound by honor and  oath to protect. 

We proclaim the freedom to choose for ourselves the types of labors, products, services,  therapies, and self-help modalities that we think best. We encourage our members to  perform their own research by studying different resources to prevent illness, as well as  dis-eases of our minds and bodies, and for achieving and maintaining optimum wellness.  We proclaim and reserve the right to include any and all options within mankind that  include but are not limited to cutting edge treatment modalities and therapies practiced  or used by any types of healers or therapists or practitioners the world over whether  traditional or nontraditional, conventional, or unconventional.  

Specifically, the mission of The Church and its outreach ministry is to change existing life  and wellness circumstances through teaching alternative wellness awareness, which  enables members to improve their physical well-being and to provide members with the  highest-level services and of research and the most effective modalities for prevention. 

The Ministry as above will recognize any man or woman (irrespective of age, race, creed,  gender, sexual orientation or religion) who has been approved as a member and acts in  accordance with these principles and policies as a member and will provide a medium  through which its individual members may associate for actuating and bringing to fruition  the purposes heretofore declared. 

NOTICE: The Church and its outreach ministry assign words and definitions, and no  presump:on can be made otherwise. We shall look to Nature and Nature’s God for such.
